By signing below I/We agree that by signing this “Notice to Vacate Form” that I/We agree to release possession of the apartment being leased by me at the address and on the date listed above. In the event that I/We do not physically turn over possession by turning in the apartment key(s) to one of the HACF Offices or Freeport Housing Authority Staff, I/We agree to allow the Freeport Housing Authority to change the locks, dispose of all items left in the apartment and re-lease the unit. I/We agree that the Freeport Housing Authority and/or Freeport Housing Authority Staff will not be held responsible for any misplaced, damaged/broken, or discarded items left in the unit after the agreed upon move-out date listed above has passed. I do hereby give consent to the Freeport Housing Authority to donate any of the possessions left in my apartment upon vacating to charitable organizations (ex. Goodwill, Salvation Army, etc.). I give consent to the Freeport Housing Authority maintenance personnel to conduct a move-out inspection during the last week of my tenancy (Monday - Friday, between the hours of 8am – 4:30pm) prior to my move-out date to inspect my unit and check for any possible needed maintenance repairs. I understand that it is not required that I be home during this inspection. If no one is present at the time of inspection the Freeport Housing Authority Staff will inspect the apartment and leave a notice stating the date, time of the inspection and a signature of who inspected the apartment.
